Sending Consultation Cases through Telemedicine

All submissions MUST be accompanied by a completed AFIP Consultation Request Form (AFIP 288-R). This form may be completed online, or may be downloaded and FAXed to (202) 782-9010.

No cases can be accessioned without a completed AFIP Form 288-R. Please DO NOT label your images with any patient-identifying data (name, SSN, etc.). Use of the local surgical number as a label is allowable.

Via secure WWW server
Case submission can be accomplished online by using our secure (128-bit encryption) WWW server.

Click on the following link to fill out our online AFIP Contributor's Contributors Consultation Form. Please fill out all fields marked with an asterisk(*), to include your FAX number. All consultation results are returned to the contributor by FAX. If you are submitting from outside of the United States, please include your telephone country code.

When you have completed the online form, please press the button marked "Submit Form and Proceed to Image Upload Page". Your case will be assigned a "Telemedicine Tracking Number (TTN)" at this time.

Next, click on the link marked "Attach images to secure email". An empty, pre-addressed email wrapper should appear in your browser. Copy the TTN which was assigned to your case into the Subject: line. This will allow us to match your submitted form with your case images.

Upload via FTP Account
For contributors with FTP (File Transfer Protocol) capabilities, a passworded account will be created, at your request, on our ftp server.

After a contributor's account is created on the ftp server, the account name and password will be sent to the pathologist by fax or over the telephone. Please provide telephone number(s) and fax number(s) where the final report may be sent. Please note: for security reasons, we cannot send logins and passwords by e-mail.

The images can then be transmitted through FTP using FTP software available on the local system.

Attach case images from your hard drive to the email wrapper and send.

As the Internet should not be considered a secure mode of transfer, please do not label images with any patient-identifying information (patient name, SSN.) Use of local surgical numbers (i.e., S-96-1092A.#1, S-96-1029A #2, etc.) is allowable.

Following receipt of the AFIP 288-R and case images, the case will be accessioned and reviewed by the pertinent Pathology Branch and the final diagnosis will be faxed to the referring pathologist within 24 hours (usually within 1-4 hours.) The AFIP consultant may communicate with the referring pathologist when more images or information are necessary.
